People with cardiac disease who have type 2 diabetes have higher hospital readmission rates (22%) compared to those without diabetes (6%). Although self-management is an effective approach to improve quality of life, there are no programs specifically designed for patients with these dual conditions. Furthermore such programs need to be enhanced by user-friendly current technology and supported by patient peers. This project aims to extend the development of a Cardiac-Diabetes Self-Management Program (CDSMP) and the preparation of lay personnel (peer, a former patient with similar conditions) to provide follow-up support.A randomised controlled trial will be used to explore the feasibility and acceptability of the CDSMP incorporating DVD case studies and training peers to provide follow-up support by telephone and text-messaging.The aim of this project is to develop and evaluate a model of peer support based on self-efficacy theory for cardiac patients with type 2 diabetes. All these patients will have the regular educational programs plus participation in the peer support program.
