Onsite Care Teams Enhance Access to Medical and Mental Health/Substance Abuse Services for Homeless Individuals

Summary
To meet the mental health, substance abuse, and primary/preventive care needs of homeless individuals and those at risk of losing their current housing, Penobscot Community Health Center provides onsite services at a community health clinic, with psychiatric and medical professionals offering primary care, disease management, psychiatric, and substance abuse services, along with referrals to additional mental health and other specialty services that may be needed. Data suggest that the program is enhancing access to care and helping patients achieve stable housing.
